We (me and my son) had kind of same troubles with Mitsumi FX001D and 
PAS 16 soundcard. With DMA 0 all but DOS based CD-ROM games (ie. SAM 
& MAX) did'nt start (crc101 error, not ready reading drive x) and with 
DMA 1 they started but the sound was really distorted and bad, also in 
Windows (WAW's etc.). Our solution to your first problem was (hint from 
this newsgroup by Eric Sterbenz and James L. Coffey) to disable DMA 
(software interrupt) ie. changing the T parameter in the 
device=mtcmcdae.sys to /T:S.
